Environmental Accessibility Services
Home and vehicle adaptations necessary for the health, welfare, and safety of waiver participants — enabling greater independence in the home and community. All work is coordinated through the participant's case manager and delivered consistent with prior authorization and program rules.
Common Covered Modifications
Ramp Installation
Modular and constructed ramps providing wheelchair and mobility-equipment access at home entrances — the most frequently authorized accessibility modification.
Grab Bars
Strategically placed, reinforced grab bars in bathrooms, hallways, and transitions to support safe movement and transfer.
Doorway Widening
Widening interior and exterior doorways to accommodate the participant's wheelchair or other mobility access equipment.
Bathroom Facility Modifications
Configuration of bathrooms for accessibility — including shower and toilet access, where necessary to accommodate the participant's equipment and needs.
Specialized Electric & Plumbing Systems
Electrical and plumbing adaptations necessary to accommodate medical equipment and supplies required for the welfare of the individual.
Other Authorized Adaptations
Additional adaptations authorized by the department that are necessary for the health and safety of the participant and not otherwise covered under the Medicaid state plan.

Prior Authorization
We understand services must be prior authorized by the department before work begins. We provide documentation, measurements, and scope details your agency needs to support the authorization request.
Enrolled Provider Standards
Work is completed consistent with the enrolled-provider requirements of the CFI (He-E 801) and DD (He-M 524) waiver programs.
Lead-Safe Practices
EPA RRP certified for lead-safe renovation — essential for pre-1978 homes, which covers much of New Hampshire's housing stock.
